When seeking a faculty advisor it is a good idea to have a clearly developed plan of study that relates to your overall academic program. Once you submit this form your faculty advisor will be emailed for approval, therefore, it is advised that you meet with your advisor prior to submitting this form. Once you have been authorized to register, you will be sent an email with instructions on how to enroll.
